Water baptism is a joyful and public way to express your decision to follow the Lord Jesus Christ.

“Repent and be baptized every one of you in the name of Jesus Christ for the forgiveness of your sins, and you will receive the gift of the Holy Spirit.” —Acts 2:38

Baptism is an identification event. Going under the water symbolizes the death of Christ. Coming up out of the water represents the resurrection of Christ. When we go under the water, it’s symbolic of the fact that we have died to ourselves and self-centered living. When we come back out of the water we are symbolizing our new life in Christ. God has forgiven all our sins and we have been given the gift of the Holy Spirit living inside of us!

From now on, if you let the Holy Spirit lead your thoughts and actions, and you build a deep relationship with Jesus, you will have the power to live a great and fulfilling life. You will not be perfect, but Jesus is perfect, and He will shine through you!

By signing below, I agree to participation in Water Baptism at the date and location noted above. I certify that the participant is physically able to participate in these activities including but not limited to being baptized by submersion.

In recognition of the risk of injury while participating I hereby for myself, the participant, my heirs, executors, administrators, assigns, or personal representatives knowingly and voluntarily enter into this waiver and release of liability (“Agreement”) and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of participation, and do hereby release and forever discharge Special Touch Ministry, Inc. and its affiliates, managers, members, agents, attorneys, employees, volunteers, heirs, representatives, predecessors, successors and assigns, the event site and its agents, attorneys, employees, volunteers, heirs, representatives, predecessors, successors from any and all liability, claims, demands, damages, actions, or causes of action now existing or which hereinafter may arise as a result of participation, whether any injury is caused by the negligence of the hosts, the participant or third parties, the conditions of the facility or any other cause.

I understand and agree that contact information, including email address, submitted to Special Touch Ministry, Inc. may be added to mailing and contact lists to be used solely by Special Touch Ministry, Inc., leadership, programs and services, to provide updated ministry communications and that I may opt out at any time, by submitting an electronic or written request for removal.

I have read this document, and I am signing it freely. I understand the legal consequences of signing this document and I agree to be bound by the terms of this document. I acknowledge that I am either the participant or parent or guardian of the participant and legally responsible to agree to these terms on behalf of the participant.
